Position Summary:
We are seeking a compassionate and dedicated Licensed Practical Nurse (LPN) to join our Home Care team. The LPN will provide skilled nursing care to clients in their homes, following individualized care plans under the supervision of a Registered Nurse (RN) or Clinical Supervisor. This role ensures high-quality, safe, and patient-centered care to promote clients’ health and well-being.
Key Responsibilities:
- Provide direct nursing care to clients in their homes, including medication administration, wound care, and vital signs monitoring.
- Follow and implement individualized care plans developed by the RN or supervising clinician.
- Monitor and document clients’ health status and progress.
- Communicate effectively with clients, families, and the care team regarding care needs and changes.
- Maintain accurate and timely documentation in compliance with company policies and state regulations.
- Ensure adherence to infection control protocols and safety standards.
Qualifications:
- Current and valid LPN license
- CPR/BLS certification (required)
- At least 1 year of nursing experience preferred (home care experience is a plus)
- Strong clinical, organizational, and communication skills
- Compassionate and patient-focused approach
Working Conditions:
- In-home patient visits
- Flexible schedule may be required (including evenings/weekends based on client needs), Part time only
- Requires reliable transportation
