Position Summary:
ComForCare Home Care is seeking a compassionate, organized, and client-focused Client Liaison to join our team. This role will serve as the vital link between our clients, their families, caregivers, and the agency, ensuring high-quality service delivery and client satisfaction throughout LaPorte and Lake Counties.

Key Responsibilities:
- Identify, develop, and maintain strong relationships with referral sources, including hospitals, skilled nursing facilities, rehabilitation centers, social workers, physicians, case managers, and community organizations.
- Develop and execute a strategic business development plan to increase referrals, client base, and brand visibility.
- Attend and represent ComForCare at community events, networking opportunities, health fairs, and professional organizations to promote services and build brand awareness.
- Track and analyze sales activities, referral trends, and conversion metrics using CRM systems to inform decision-making and improve outcomes.
- Collaborate closely with the client care and intake teams to ensure seamless onboarding of new clients and exceptional service delivery.
- Maintain an in-depth understanding of ComForCare’s services, differentiators, and compliance standards to effectively communicate with referral partners.
- Provide market feedback and insights to inform business strategies, service offerings, and competitive positioning.
- Prepare and deliver presentations to referral sources and stakeholders, highlighting ComForCare’s value proposition and success stories.
- Consistently meet or exceed monthly and quarterly referral and revenue targets.
- Ensure compliance with all applicable healthcare regulations and company policies.
Qualifications:
- Bachelor’s degree in business, marketing, healthcare administration, or a related field preferred.
- Minimum of 2-3 years of experience in business development, healthcare marketing, or sales, preferably in home care, hospice, or related healthcare services.
- Strong communication, presentation, and interpersonal skills, with the ability to build trust and rapport with diverse stakeholders.
- Proven track record of achieving business development or sales targets.
- Ability to work independently, manage time effectively, and travel extensively within LaPorte and Lake Counties.
- Familiarity with local healthcare systems and community resources in the coverage area is a plus.
- Proficiency in Microsoft Office and CRM tools.
- Valid driver’s license, reliable transportation, and proof of auto insurance.
Working Conditions:
- Regular travel within LaPorte County and Lake County.
- Flexible schedule required to attend events, meetings, and networking opportunities, including occasional evenings and weekends.
